PATIENT REFERRAL COMPETITION

WIN A Weber Q BBQ!

Conditions of Entry

Promoter is Smiles Only Pty Ltd. ABN 85 632 125 314

Prize Details:

The winner will receive ONE WEBER Q BBQ (Model no. 3400046, Weber Q 2600N+ LP Flame Red) and ONE WEBER Q STAND (Model no. 3400175, Weber Q Premium Cart) and ONE Precision Grill Tong and Spatula set (Model no. 6771).

The winner must supply sufficient identification details to confirm their identity before the prize is granted. The prize is not exchangeable and cannot be redeemed in cash. In the event of unforeseen circumstances, Smiles Only Pty Ltd, reserves the right to cancel, terminate, modify and/or suspend the Competition or suspend or modify a prize.

Duration of Competition:

Competition start date: 23 October 2023
Competition end date: 22 March 2024

How to Enter:

Competition is OPEN to all existing patients of Brisbane Smiles who are sixteen (16) years of age or above. Existing patients are clarified as any patient who has attended Brisbane Smiles within the past 3 years (since 23rd October 2020).

Employees of Smiles Only Pty Ltd and their immediate family members are excluded from entry to the competition.

Only valid entries will be accepted. A valid entry is an original ‘referral card’ with the referring patients name filled out and dated.

Each ‘Existing Patient’ can be given more than one ‘referral card’ and can have multiple entries into each ‘Referral Competition’ draw.

One (1) valid entry per New Patient. New Patient cannot present multiple referral cards from multiple patients. Only one (1) card will be accepted.

One (1) Referral Card must be presented at the first New Patient appointment and given to reception. Reception team members will put the valid ‘referral card’ into the ‘Referral Competition’ draw. All ‘Referral Cards’ will be recorded.

$50 for adult patients and $25 for child patients (15 years and under) will be taken from the new patients first appointment total upon presentation of the patient referral card. This card is not transferrable for cash, no change is given and the full amount is redeemed at one (1) appointment only.

Existing patients who refer a New Patient to the practice who presents a ‘Referral Card’ will receive one (1) entry for each new patient they refer.

All entries must be received within the competition period to be valid.

There is no maximum number of entries. All entries have an equal chance of winning. The winning entry is drawn on chance.

Prize Draw:

Prize will be drawn at random and recorded at the same time.

Competition will be drawn randomly at Brisbane Smiles, Level 1, 31 Sherwood Road, Toowong by Dr Paul McKay and recorded at 5.30pm March 22nd, 2024.

The Winner will be contacted via our contact details provided in our practice software and may be communicated via our social media platforms.

The winner must contact Smiles Only via our phone number 07 38703333 or email [email protected] to claim the prize within five (5) business days of the prize draw.

As a condition of entering this competition, each entrant agrees to give permission for Smiles Only to use the entrants details for the purpose of promotion on our website, social media and other electronic or printed communication emails. You agree to participate and co-operate as required if any editorial request is asked, for example being photographed or interviewed, and grants Smiles Only a royalty-free licence to use such footage/photography for any media for an unlimited time.

The prize must be picked up in person from our Toowong practice (Level 1, 31 Sherwood Road, Toowong). Installation of the ‘prize’ is not included.

The prize is on display in the waiting room throughout the competition period and will be supplied with the original packaging. No liability is accepted by Smiles Only for damage to the prize during the competition period, and during transportation.

If the winner is unable to be contacted, or is unable to accept the prize, then it is at Smiles Only discretion to redraw the winner.

This competition is not endorsed or associated with any brands or other associations and represents Smiles Only Pty Ltd, trading as Brisbane Smiles.

Exclusion of Liability:

Smiles Only is not responsible for entries not received, or for New Patients not presenting ‘Referral Cards’.

Smiles Only will not be liable for any loss, damage, or personal injury sustained in association with this competition or the acceptance of the prize.

The winner bears all responsibility for any extra costs of this prize, including but not limited to, maintenance, insurance and costs associated with installation. Once the prize has been accepted and transferred, Smiles Only is no longer liable.
