PATIENT REFERRAL COMPETITION

WIN A $500 BP FUEL VOUCHER!

Conditions of Entry

Promoter is Smiles Only Pty Ltd. ABN 85 632 125 314

Prize Details:

The winner will receive ONE $500 BP FUEL VOUCHER.
The winner must supply sufficient identification details to confirm their identity before the prize is granted. The prize is not exchangeable and cannot be redeemed in cash. In the event of unforeseen circumstances, Smiles Only Pty Ltd, reserves the right to cancel, terminate, modify and/or suspend the Competition or suspend or modify a prize.

Duration of Competition:

Competition start date: 1st June 2022. Competition end date: 30th November 2022.

How to Enter:

Competition is OPEN to all existing patients of Brisbane Smiles who are sixteen (16) years of age or above. Existing patients are clarified as any patient who has attended Brisbane Smiles within the past 3 years (since June 2018).

Employees of Smiles Only Pty Ltd and their immediate family members are excluded from entry to the competition.

Only valid entries will be accepted. A valid entry is a completed referral form with the referring patients name with email correctly filled out. One (1) valid entry is per one (1) referred patient name and email.

Each ‘Existing Patient’ can be complete a referral form more than once and can have multiple entries into each ‘Referral Competition’ draw.

All entries must be received within the competition period to be valid.

There is no maximum number of entries. All entries have an equal chance of winning. The winning entry is drawn on chance.

Prize Draw:

Prize will be drawn at random and recorded at the same time.

Competition will be drawn randomly at 12.30pm Tuesday 6th December, 2022 by Dr Paul McKay and recorded.

The Winner will be contacted via our contact details provided in our practice software and may be communicated via our social media platforms.

The winner must contact Smiles Only via our phone number 07 38703333 or email [email protected] to claim the prize within five (5) business days of the prize draw.

As a condition of entering this competition, each entrant agrees to give permission for Smiles Only to use the entrants details for the purpose of promotion on our website, social media and other electronic or printed communication emails. You agree to participate and co-operate as required if any editorial request is asked, for example being photographed or interviewed, and grants Smiles Only a royalty-free licence to use such footage/photography for any media for an unlimited time.

The prize must be picked up in person from our Toowong practice (Level 1, 31 Sherwood Road, Toowong).

If the winner is unable to be contacted, or is unable to accept the prize, then it is at Smiles Only discretion to redraw the winner.

This competition is not endorsed or associated with any brands or other associations and represents Smiles Only Pty Ltd, trading as Brisbane Smiles.

Exclusion of Liability:

Smiles Only is not responsible for entries not received.

Smiles Only will not be liable for any loss, damage, or personal injury sustained in association with this competition or the acceptance of the prize.

The winner bears all responsibility for any extra costs of this prize. Once the prize has been accepted and transferred, Smiles Only is no longer liable.
